This episode sets the stage for much larger things to play out.

Story: An OD case in the ER has Kincaide on edge. Devon’s clinical trial doesn’t get off to a great start. Randolph’s first day on the state medical board gives him a clear idea what he is up against. Devi makes a Conrad-level medical diagnosis that saves a patient’s life and begins to give serious thought to Padma’s request.
Review: It was about time that The Resident addressed the mystery that was Kincaide Sullivan. Turns out, she is working with the FBI on cases involving Medicare fraud and that’s put her on the mafia’s hitlist. No wonder she doesn’t dole out much info about herself. And now, Conrad’s involved too. Someone’s got all of Conrad’s details and has been handing out prescriptions for a couple of months and one of the recipients ends up dead in the Chastain ER with anti-psychotics allegedly prescribed by Conrad. As a doctor who also works undercover for the FBI, Kincaide also has to be on the run and will up and leave soon enough, so, there’s no chance for any budding romance between her and Conrad. So, that’s been nipped in the bud or has it?
Randolph/Dr Bell has joined the State medical Board, but it doesn’t take him long to realize that his fellow members want to do precious little and not take action in cases of genuine negligence by some doctors. Famous doctors are well-connected in the board, which ensures they are not held accountable for their actions. Randolph, though, wants to help the lady who became a paraplegic after an avoidable mistake in surgery. We’ll have to wait to see what plan he comes up with.
Back from his directorial duties, Manish Dayal as Devon has his work cut out for him. A cancer patient, for who his clinical trial is a last resort, is not taking it very well and the worse he gets, the higher the chances of the trial to be temporarily abandoned. Devon wants to give his patient a shot at some more good days, but at what cost? Turns out that’s not the only worry for Devon on this episode. After figuring out a miraculous diagnosis that saved her patient – a go-getter businesswoman, for who career comes first and has no desire for kids – Devi is all set to donate an egg for her twin sis Padma to have a baby by IVF. Devi will freeze a few for possible use down the line, but she’s not there yet and doesn’t know if and when she will be. For now, this is all she can promise Devon, and shuts down his dreams of having a family with her. And considering that Devi is the rising star in Billie’s and AJ’s ORs, it won’t be long before she and Devon become history, we think.
Verdict: Apart from the routine stuff in the ER, this episode was more a set up for things to come. If Kincaide is still being hunted by the mafia, now that Conrad’s gone to the FBI, there will probably be a target on his head too. That would be interesting to see how he navigates his way out of this.
