The horror-thriller genre is not widely explored in Malayalam, though it has occasionally been given fresh perspectives. While some films contained oblique allusions to Hollywood ones, others tried impressing the audience with relatable subjects and jump scares.

Aparichithan

Even though the Mammootty-starrer film struggled at the box office, it was successful in frightening people by introducing the idea of the Ouija board, which the Malayalis were not familiar with in the early 2000s. The most frightening aspects of this film are the partially burned face of Mahhi Vij's character Kalyani and the ending revelation about the deceased, which stays with you for a very long time.

Manichitrathazhu

This Fazil film is among the most rewatched Malayalam films of all time. A must-see for its performances, Manichitrathazhu is a psychological horror film, with Shobana's transformation into Nagavalli standing out in particular. Despite being adapted into multiple languages, these films failed to make the same impression as Manichitrathazhu.

Moonnamathoral

In 2006, this film may have caused more confusion than fear in many viewers. But it has managed to do the genre it dealt with justice, adding the necessary details like a big house situated in a remote area and a moody female character who could be the perfect target for a wandering spirit to possess.

Vellinakshatram

The 2004-film Vellinakshatram, starring Prithviraj Sukumaran, was successful in frightening a number of kids by showing a child and the female lead as being possessed by ghosts from past lives. Despite its scary scenes, the movie never fails to keep audiences glued to the screen whenever it airs on television.

The Priest

The Mammooty movie featured every cliche associated with a horror thriller, such as jump scares, possession of a vulnerable person, and a tragic past for the spirit seeking retribution.
