In Julia Whelan's novel, Jamie is temporarily released after contracting pneumonia and participating in a clinical trial, which allows him to fulfill his promise to travel to Europe with Anna, who is known as Ella in the book.

However, the film takes a more somber turn as Jamie's health begins to deteriorate. Towards the end, Anna finds Jamie unconscious next to her in bed. She rushes him to the hospital, where she faithfully stays by his side as he reaches the end of his life. After his death, she imagines that he will accompany her on her European travels - until those dreams slowly fade, leaving her to face the journey alone.
