Baaghi 4, directed by A. Harsha, looks brutal and violent in the trailer, but when you watch the film, the plot looks broken. The fourth instalment shows Tiger Shroff as Ronny meeting with a train accident while he was in a car. However, he dramatically survives, and months after being declared brain dead, he recovers. But something has changed as people around him, including his doctor, his brother, and others, make him believe that he is hallucinating.
When he was in a coma, he mumbled one name, and that is Alisha (Harnaaz Sandhu). She is a doctor with whom Ronny fell in love. Alisha was in the car with him, and Ronny thinks she has died in the accident. Whenever he talks about Alisha to his people around him, he is said to be hallucinating, and there is no one called Alisha. But he knows Alisha was there in his life, and the trauma of losing his beloved makes him aggressive. Ronny's brother Jeetu (Shreyas Talpade) sends Olivia/Pratishtha (Sonam Bajwa) to cheer him up, but they become friends.
Later, Ronny realises that his picture with Alisha has been replaced to make him believe that he is hallucinating. After a long action scene, Ronny finds out that everyone conspired against him. But he cannot figure out who the mastermind is behind it. Amid this, Olivia informs Ronny that Alisha is alive. Meanwhile, Jeetu loses his life while protecting Ronny from Chacko (Sanjay Dutt) and his men. Alisha's fake death was orchestrated by a sinister man named Chacko. Chacko has hold Alisha hostage.
Chacko is the mastermind behind all this hallucination and Alisha's fake death scenarios. He wants Alisha because his girlfriend, Avantika (also played by Harnaaz Sandhu), is a confident woman. But on the day of their wedding, Avantika is killed during a violent incident. Chacko challenges 'God' to bring her back. After the accident, he discovers Alisha. Obsessed with her, Chacko secretly rescues Alisha by causing the accident.
Chacko, who holds Alisha hostage at his mansion, desires to marry her. But Alisha denies and says that she loves Ronny and cannot pretend like Avantika. In the final scene, Ronny, who was tied by a chain, makes it in time to stop Alisha and Chacko’s marriage.
A huge, violent fight ensues. Chacko believes he has pitted Alisha against Ronny, but their love wins. During the scuffle, a pillar in the Church cracks, and just as Chacko is about to shoot, the pillar collapses on him, killing him instantly. Later, Ronny and Alisha get married, leading to a happy ending.
